After filling out all necessary documents, signing all the documents where specified, please either FAX them to +81-3-5217-0617 or E-mail them to Customer Care for varification.
Please note: Only documents printed in black ink shall be accepted from the online signup for Live Private Accounts. If documents are printed on a colour printer, the document must resemble the document colouring on the online sign up page. Documents printed in all blue or any other colour other than black with highlighted words in red shall be rejected.
*Legal documents in Japan can only be accepted in black ink or black ink with highlighted words in red.

A copy of a recent utility bill that is less than 3 months old.
Either a gas bill, electricity bill, water bill or a telephone bill, as clear to read as the one below.
The utility bill must show the following clearly.
- Your residential address,
- Name of issuing corporation,
- Contact information (address, telephone number etc.)of issuing corporation.
- Date of issue.
( If the utility bill is in a language other than English, we will need you to submit a full clear black and white certified or notarized translation in English of the document, along with the date and your signature. This will require the assitance of a local lawyer or notary public official for certification of the document. The Lawyer or Official must also state their name, address and contact information, add the date and sign the certified document)
